After the helicopter has split Lara and Jonah up, there’s a stealth combat section. After the two sections of combat, this journal is lying on the ground when you climb up out of the second zone.

In the Porvenir Oil Fields, this is in the area right after Lara tells Jonah to get off the radio channel, but of course it’s still there later at this location if you need to come back.
