Well in Skyrim, if you hold A/X on an object, you can physically pick it up without adding it to your inventory, and you won't get in trouble... C'mon guys.

The reason why Altair can't swim is because of a glitch in the Animus.. there's a message or note in the animus that talks about the glitch... I'm sure you guys haven't missed that, right?

Fable's house management system comes to mind. If a landlord in real life is negligent in his/her duties to the property they rent out, then yeah, I can see it looking a bit shabby. But, in Fable III, if you forget to repair your houses for a week or so, they turn into backdrops for an apocalypse movie. Seriously, I forget to fix the sink and all of a sudden you board up the house CoD zombies style and stop paying rent?

Dying in water always seems to be in the early game or two, then you learn to overcome it in later games.

hasn't the water death been in basically any and every game since the beginning of time, except for maybe Wave Race? Also, Dead Island makes complete sense. If I were to smack someone repeatedly with a crowbar, eventually it would turn into a state where it's like hitting them with jelly. What's not to believe? How about being shot all over your body, having a smidgen of health left and then you die from jumping from a ledge that's just too high or being tapped by a vehicle, or using one of your examples, making Snake have a cigarette? Get hit by a tank shell, get shot by a minigun and a Harrier jet and then die from one drag on a cig. Makes sense to me

I'm surprised that the first aid healing in MGS3:Snake Eater wasn't even mentioned. I mean, the game does go one step further in realism in the sense that if you're bleeding, you need to bandage or cauterize the wound, if you break a limb, you need to put a splint, if you get a burn, you need to treat it.... But the realism ends the moment you bandage it and you heal from it almost instantly XD

I actually really enjoyed the weight system in GTASA.. and all the other RPG elements of it. Probably why it's still my favorite GTA game to date. And I feel like RDR should be a target for the swimming statement. You can literally be standing in the water near Blackwater, and if the waves of it slap up to just over your mouth, Marston dies. Instantly. Pretty damn funny.

The assassins creed explanation for the killer H20 was from an animus glitch, think it was in the game manual, but still he may as well be a berocca when it happens
