In Ocarina of Time I got stuck in the Water Temple, like so many others. This must've been when I was like, 10, though. I used to have a really hard time with puzzles in games when I was young.
Also, in Starfox Adventures, I got stuck on the final boss, Andross (SPOILARZ!). After a couple days of trying to beat him, I gave up and used my Action Replay to get invincibility. That fight really made no sense, because you spend almost the whole game on foot, and then one of the hardest parts of the game is in the Arwing, which you have little practice using.
Another part of that game that always gets me is the part where you have to do the strength competition against a big lizard guy (a "Lightfoot" dinosaur), which actually means "who-can-tap-the-A-button-the-fastest" competition. I suck at those. I had to get my brother to do it the first time I played it, and then when I replayed the play the game again in November, he beat it again. But a couple days later I had to do the "Fear Test" to get a Krazoa Spirit, in which you have to keep a sporadically moving line in the center of a bar. I couldn't beat it, so I put down the controller and never picked it up again. I love Starfox Adventures, but it just goes to show how a couple cheap shots is all it takes for you to quit a game.
EDIT: And I'm with Gourdmaster on Bioshock, it also figuratively scared the fecal matter out of me. I played it for about eight hours, then deleted my save so I couldn't go back and play it. And then, of course, I started playing it again, and did the same thing. But then I gave it back to the friend I had borrowed it from. That game ******* traumatized me.

GTA IV, Assassins Creed, Portal, Half life 2, Oblivion and Fallout 3. Always end up getting distracted with other games and pushing them down my to play list.
Twilight Princess: 30hours in and my data corrupts. Not once but twice. I couldn't bring myself to complete it.
Metroid Prime 3: Too lazy and distracted.

The Zelda series of games for sure, I think I played all of them (Except the CD-i ones) and did not finish one. I would get stuck on a dungeon puzzle (Water Temple I'm looking at you)and just get too frustrated to continue.

For me it's A Link to the Past. I got so close when I was a kid, and then never beat it. And then my mom beat it and I lost interest. I've tried to go back and replay it on emulators... I absolutely love the game, but I've just never finished it. Guess something always comes up.
Bioshock too. My fiancee beat it. I watched her play a lot of it, and didn't really want to play a game I'd already seen. But everyone keeps telling me I gotta play it for myself. Just can't seem to muster up interest anymore.

That would be Elder Scrolls, Oblivion. Halfway thru I got turned off by the "monsters level as you do" gameplay. I'm sure there are mods out there to fix that, but I never got around to it. It was a great game, but some of the vanilla bugs ruined my experience to the point of uninstalling it. Horses that made everything lag, guards that caught me stealing when they weren't even around...etc.
I beat Fallout 3 multiple times, its a shame Oblivion didn't have the same leveling mechanic or I would have stuck with it.

Xenogears for the original playstation. Very much like Star Ocean 2, if you do not stack your characters from the beginning of the game or upgrade your mechs enough, or the right way...you WILL NOT beat the final boss. I've been stuck on the final boss for about 10 years now, give or take.

Two games on my list of shame. On the Super Nintendo version of F zero I just couldnt finish the last race. I tried it with all four cars but it didnt matter I couldnt do it. My younger brother even did the first two laps for me but Id aways mess up the last lap. It still bugs me a bit now because I take my racing games really seriously.
Second on the list is Zelda A link to the past. Gannon put the smackdown on me, I was too lazy to put the effort in to beat him. Eventually the save got deleted and I stopped caring.
The super nintendo gundam fighting game was almost on this list but I spent 2 hours beating the boss Gundam Epyon.

There's a huge list of games I have yet to beat, the reason being because when something new releases that I want to play, I stop playing everything else until I either get stuck or bored with the new game. Then I'll go back and play some of the older one's, half of the time starting over because I either forget what's going on in the game, or I just want to change my stats or skills if its an RPG.
The game that stands out the most for me would be Final Fantasy 8 for the Playstation. I played that thing for probably about a year, eventually using the guide book, and still never beat it. I guess the reason I didn't beat it back then was that I moved, and after we had everything situated at the new house I just never went back to it.
The reason I'm most ashamed / pissed off for not beating FF8 isn't only because I didn't beat the game, but because I was so close to beating it (I guess a few boss fights away??). I don't remember much about the game now, seeing as I haven't played it in about 7-8 years. All I remember is that my last save was in some room with a bunch of picture frames, and when you touched one of those pictures, you started a boss fight. I had mentioned this to a friend of mine a few years ago, and he was like "Dude, you were so close to beating that game, why didn't you just finish it?"
One of these day's I'll play it again and hopefully finish it. For now, I've got about 15 other games to beat.

There are a lot of great games that I have played that I've never finished but the most pathetic is the original Mario on the NES. It game with my NES in when I got it but I never beat it to this day.

I rarely let classics go unbeaten, at least for more than a year or so. But on game that I do know I never got around to beating was the latest Prince of Persia. I just got so tired of doing the same thing over and over again that I just stopped and replayed Bioshock. Also, Okami is so long I often play half of it, then come back like a year later to finish it.

I have two...Both of which I am immensely ashamed of...Probably lose a couple forum buddies for it...Here goes:
#1: LoZ: Ocarina of Time... Not only I have never beat it, I have never played past the first boss O_o I'm not sure what it is, but I lost interest when I was younger after the Deku Tree and tried it again later in life and same thin exact thing: Beat the Deku Tree boss and stopped playing...
I swear, I WILL play again one day, probably...
#2: Okami... Same kind of situation, beat Orochi and I figure the game is over but it's not...It is for me though, I have had 2 game saves in the exact same spot...The save after beating Orochi...Sorry Clover, but I bought your game, that still counts, right?

Super Mario Bros. 3 I have never got past that god damn castle maze in world 8. I just gave the **** up. I mean, come on!! The one time I thought I had it right about a year ago, I ran out of time. World 8 reminds me so much of hell... that one castle will be my eternal damnation...
