I start at all madden, normal or other levels. It all depends on the game.

Generally the default difficulty...
One thing I hate, is the difficulty in Oblivion and the fact you can change it mid battle and game...

I usually just pop in the game and start playing, I don't usually bother with changing difficulties unless I want the unlockable if I beat it on hard o whatever

I normally start on, well, normal. If I think the game's going to kick my ass, I may start on easy to get a feel for it.
A game like Oblivion, though, I always play on easy. Not because it's especially difficult, but out of principle. An arrow to the head should have some effect, really.

I usually stick with the default setting. Unless it's Contra. Then I just don't play it. Lol.

Depends on the game.
Fallout for example, I had to start out on easy, then after a few missions shot back up to normal, then hit the level cap and bumped it up to hard. The super mutants Goliaths are dam near impossible in hard unless you have a high explosive skill.
Call of duty 4 I Systematically worked my way from easy to insane.
But normally I just play on the default level.
And I almost never beat gameĀ?s on insane, just because I,
1: donĀ?t have enough time to devote to it
2: I get pissed easily and I start tossing shit around
3: I stop caring about a story after the second play through.

Call of Duty: Veteran
Rhythm games: Hard
Pretty much everything else I play on normal. However, since I am an achievement whore, I was forced to play all of Doom's levels on Nightmare.

It's totally dependent on the game. If its a game that I know is going to be hard than ill play on normal or easy if it's games like rock band than I know I can play on expert and hard.

I work my way up from the bottom except for in one game, bioshock. Its so easy anyway, and you respawn like 12 ft away whenever you die with no progress lost

I always start on chuck Norris difficulty. If that's not available (which it isn't in most games) I just downgrade to expert
|

i start on medium. i want to be able to finish the game without much effort, (because i'm the kind of person that wants to experience whatever the narrative has to offer) but still not easy enough to breeze through. if the game is worth it, i'll beat it again on a harder setting.

yah i totally agree with penguin. i play my first playthrough on normal so i can expierence the story. because if i do the hardest right away theres a chance ill get too pissed off and then never want to play.,
