In my almost 12 years of playing Golden axe on the genesis, my brother and I have never beaten it, im ready to throw it at the wall (however me and a friend finally beat streets of rage for the first time out of 12 years of trying. my brother is still pissed that he can't say he's beaten it.)
I think the worst for me was the fact that even though it was one of the first games that I'd EVER owned, and EVER played...I didn't beat Super Mario Bros. for the NES. Not until it came out for the Game Boy when I was in the eighth grade. I've beaten lots of other games though. Just...y'know, I had other stuff to do.
Chrono trigger, an absolute masterpiece. always get to the part in which you are in the earth's crust about to fight lavos, but while traveling through the annoying lava-themed dungeon, I am almt always killed before I can reach a save point and put the game down, frustrated about losing alot of progress (Is it really that hard for a "Retry" function to be implemented?). I return to it later, but since I haven't been invested in the plot or combat for awhile, I feel it necesscary to start all over again.
I never beat Zombies Ate My Neighbors on the SNES, I always died on the fat baby,(keep in mind that I was only 6 years old) and my SNES is broken now so I'm never going to be able to finish it, ever! =(
I'd have to say Fallout 3. And maybe Hitman 2. I quit fallout early because I got frustrated by how many times i kept getting prawned by raiders and super mutants. Hitman two just got outrageous. (Jogging busts your ass. Shoot all of the million guards in the f'in church.) No, and no. And, to my eternal shame... I have never finished MGS1.

4. The orginal Doom
5.(going current gen) Prince of Persia
6. Burnout Pardise
7. Assassin's Creed
8. Far Cry 2
9. Progect Gotham Racing 3
10. And finally, until two days ago, I had never beat Mass Effect
That's only the first ten, there are around twenty more from the PS1 gen.
any of the three Prince of Persias for last gen. I'm not as ashamed for not beating those games as I am for beating my xbox as a result of frustration.
nevr beaten secret of mana. i downloaded it on wii thinking it was gonna be awesome and it was, just didnt have the time to play it. Then i downloaded megaman 2 and punchout and got sucked into them.
i still highly recommend playiong and finishing secret of mana
Lol its actually embarrassing since it was a good game, Bioshock. I visited my ol' roommate one weekend and I brought my 360 along with Bioshock and a few games. Played the crap out of it, got past the "would you kindly" part and started to play other games once I got back. Just been playing everything else and with school now I don't want to jump in since it will eat up time. Bioshock is one of those games, an underwater experience, that you play for really long times or not at all.
Resident evil 1 on the sega saturn. i gave up on the game after my memory cartrage crashed and i lost my save i was about half way through the first time it happend but it happend anouther 2-3 times so i gave up. 74th wooo
I am reaaally sad to say that although Ocarina of Time on the game cube was fantastic, I could never get through the Gerudo things because I didn't use a walkthrough
