Palazzo Laterano: The Sixth Day
Flag 1
As you're working your way through the Pope's abandoned former residence, you'll find yourself climbing on what look like hanging double doors. While you're wrestling with them, the first flag will be clearly visible on your left. To get to it, just continue making your way toward the platform where the arrow's pointing:

Once you reach it, you'll be able to head right or left. Head left across the exposed beams, and then make another left once you're past them.

The first flag should be just a quick jog away.

Flag 2
Another tease behind a grate. For now, just head to the left of it, across the exposed beams to the platform the arrow's pointing at below. Then drop down into the next room.

In the next room, you'll find a Madonna statue; climb it to reach the platform just above it.

Once you're up there, look right. The next flag is just a couple of hops away.

Flag 3
You'll know you're close to the lair's final flag when you're forced to shimmy across crumbling, hanging scaffolding that looks like this. Head left, jump over to the wall and climb up to the ledge.

Look to your right, and you should see the next flag on a wooden slope. Hop over to where the arrow's pointing and you'll be in easy reach of it.

Cloaca Maxima: Leader of the Pack
Flag 1
The first flag is in a room where you'll have to fight a handful of Romulus followers, after which your chase target will run off past an iron gate. Ignore the "reduce distance" warnings for now; he's not going anywhere.

To grab the flag, run up and climb the wall below and to the left of it. You'll want to work your way over to the handholds on the wall to Ezio's left, here:

Once you've climbed up that wall, you'll be able to jump off to the beam on which the flag sits. Easy!

Flag 2
A little further along, you'll reach a big, cavernous room that's crawling with Romulus followers (and their laundry). Find the structure that looks like this, to the right of the room's entrance, climb it, and then head across the laundry line to the next platform.

From there, you'll see an awning below a ledge patrolled by Romulus followers; hop over to it and scramble up to the ledge above, killing anyone who gets in your way..

Follow the ledge until you see this hanging pot. Jump over and grab it, and then continue forward until you reach the next wall ledge.

Follow the ledge until you see the wall pictured below, then use the swing bar in front of you to leap over to it.

Once you've grabbed hold, shimmy over to the right until you're able to jump onto a beam jutting out of the wall.

At this point, the followers guarding the flag will probably take notice of you. Jump over to their ledge and start murdering them.

Once they're not protecting it anymore, you'll have a clear path to the flag.

Flag 3
This one's pretty easy. Toward the end of the level, you'll confront the leader of the followers, and end up facing a doorway that looks like this:

Turn right, and you'll see the next flag behind a gate. Jump into the water, swim over to it and press A or X to dive below the surface just long enough to get under the bars, and it's yours.

Basilica di San Pietro: A Wolf in Sheep's Clothing
Flag 1
Soon after you start chasing the Cardinal through the Basilica, you'll eventually climb the inside of a tower with two exits at the top. Take the one the arrow's pointing at below:

You'll see the first flag on top of an obelisk outside; to get to it, scramble up that brick wall and jump off it to the right, and Ezio should grab hold of tthe obelisk's rim.

Flag 2
Further on, you'll be chasing the Cardinal through some scaffolding near the Basilica's ceiling. Follow him as closely as possible.

When he rounds the corner into the next section, leap over to the hanging lamp to continue the chase...

... then do the same again with the next lamp, directly across from where you just landed.

After you've landed from that second swing, look left. The next banner is there for the taking.

Flag 3
After you confront the Cardinal, you'll be directed to a pair of flapping curtains, past which you'll find one of those narrow viewpoint/Leap of Faith platforms. But don't leap just yet.

Instead, drop down. You'll see the next flag on a beam below you; after you grab it, scramble back up the unfinished rose window to get back on track to finish the level.

Congratulations! Assuming you've done everything as directed by this guide, you should now be in possession of all 101 flags and 10 feathers. You won't see a new cutscene or anything, but you will get a slightly disappointing 30G Achievement/Bronze Trophy. You'll also get the Borgia Cape, which when worn will ensure that your Notoriety never rises.
